Social Worker with Masters Needed for Hospital
Academic medical hospital located in Brooklyn is seeking to hire a Social Worker with at least 1 year experience in the clinical field. Internship experience is also welcomed!
Our team is looking for a passionate individual who has experience in evaluating and treating patients through an individualized program of supportive counseling and assessing.
Responsibilities will include but are not limited to:
• Evaluate patients within assigned area of social work discipline.
• Construct social work treatment plan and Conduct ongoing assessment of various factors for patients.
• Counsel patients, family and friends in adjustments to chronic illness, changes in lifestyle, and choice of treatment.
• Assist with other services to assure proper and timely referral for transient care.
• Observe and monitor transportation referrals to assure timely and efficient services.
• Communicate and confer with financial offices to provide information exchange.
• Report cases of abuse and or neglect.
• Act as a resource to staff, patients, families and/or caregivers.
Requirements:
• Master’s degree on Social Work Education from an accredited program.
• NYS Social Worker License
• Minimum one year experience as a Social Worker in a supervised setting within the last three years.
• Clear written and oral communications skills, excellent interpersonal skills for interaction.
